This pilot programme provided community-based services to assist elderly residents to age in place through the provision of a neighbourhood rehabilitation centre and a home visit service by healthcare professionals.

A new rehabilitation centre was set up to cater to elderly residents who required rehabilitation services for age-related conditions. Many of these elderly did not go for scheduled therapy sessions because of financial constraints, inconvenience, lack of caregiver support and lack of awareness. As a result, their physical functioning abilities were compromised.

There was also a home visit programme to provide medical, nursing, therapy and hospice services to frail elderly who were homebound or terminally ill. The team of doctors and nurses would visit these elderly in their homes regularly. In addition to providing medical and nursing care, they provided support and advice to caregivers, as well as therapy and palliative home visits when needed.
