The Care Close to Home pilot programme supported vulnerable seniors to age-in-place within the community. Under this programme, Care Teams consisting of Social Workers and Health Care Aides from Seniors Activity Centres in Ang Mo Kio and Kreta Ayer provided care coordination and services to vulnerable seniors living in 1 to 2-room rental flats in these two areas.

This community-based programme provided client-centric personal daily care services for the elderly. These included physical and cognitive care (e.g. medical and nursing care), social care (e.g. befriending and financial support) and personal care (e.g. showering and toileting). The Care Team also assisted to coordinate medical appointments on behalf of the elderly and escort them to these appointments.
