The CarerSupport@Centres programme aimed to establish a new care model at Eldercare Centres that supported both elderly and their caregivers, as well as help caregivers better manage their caregiving burden and sustain care for their loved ones within the community.

The pilot was implemented at five Eldercare Centres, comprising Senior Care Centres and Dementia Day Care Centres. These Centres were resourced to build up new capabilities to support the caregivers with the following services:

- Core Services (for all caregivers):
Screening for caregiver burden and needs assessment
Care coordination and casework support
One point contact for emotional and practical support
Education on caregiver resources
Consultation on Behavioural and Psychological Symptoms of Dementia

- Add-on Services (home-based and where necessary)
Assessment or intervention
Behavioural management
