The integrated care programme is trialled at the Integrated Maternal and Child Wellness Hub set up within the premises of Punggol Polyclinic. The programme comprises:
- Care coordination for at-risk children and mothers
- Training healthcare workers at the Polyclinic to provide comprehensive screening
- Screening of mothers and their children who come to the Polyclinic for immunisation. The screening includes child development, nutrition and growth of children as well as the mental health and wellness of mothers
- Community outreach to raise awareness of child development, wellness and parenting
